| All Indiana Diving Academy campers must be registered with U.S. Diving in 2007 for insurance purposes. Limited lesson membership is $30.00 and provides four months of coverage from the time of enrollment from US Diving. We are no longer being covered by the AAU Insurance for our summer camp, you MUST sign up for U.S. Diving! |

| In addition to being registered with U.S. Diving for insurance purposes, Indiana Diving requires all camper parents to provide a Medical Consent Form allowing trained officials in and around Bloomington to give medical care to the minor if warranted. |
